Job Summary

We are seeking an experienced Hospice Registered Nurse to join our team at HopeHealth, a leading provider of home health, palliative, and hospice care in Massachusetts and Rhode Island. The successful candidate will work closely with interdisciplinary teams to provide skilled assessments, complete admissions, and develop individualized plans of care for patients and their families.

About the Position

This full-time position offers a competitive salary range of $85,000 - $105,000 per annum, commensurate with experience. As a Hospice Admission RN, you will have the opportunity to make a meaningful difference in the lives of patients and their families, promoting comfort, dignity, and peace of mind throughout the end-of-life journey.

Responsibilities and Expectations

  • Represent HopeHealth in assigned settings, presenting hospice services to eligible patients and caregivers
  • Complete initial assessments to determine patient and family needs, applying knowledge of Local Coverage Determination training
  • Develop and implement individualized plans of care, ensuring compliance with Medicare/Medicaid and state regulations
  • Collaborate with interdisciplinary teams to deliver high-quality patient care

Requirements

  • RN licensure in Rhode Island and Massachusetts
  • Minimum 1-3 years of clinical experience
  • Excellent nursing assessment, clinical, and communication skills
