Location

Our Cardiology department specializes in the prevention, diagnosis, and treatment of heart disease, aiming to improve overall health. We offer a full suite of cardiology services, including general and interventional cardiology, heart failure interventions, electrophysiology, device implants, anticoagulation management, and more.

This role will support our Heart Failure Clinic, which operates under the guidance of a Medical Program Director in collaboration with Advanced Practice Registered Nurses (APRNs). Our Nurse Practitioners focus on heart failure management, adjusting patient medications, and working closely with cardiologists as necessary. Additionally, our team of nurse navigators assists patients throughout their journey, coordinating clinic appointments and serving as the primary contact for patient inquiries.

The Opportunity

We are looking for a full-time Nurse Practitioner II to join our Heart Failure Clinic team, working Monday through Friday from 8:00 AM to 4:30 PM.

As integral partners to our physicians, Nurse Practitioners have the autonomy to practice at the top of their education and certification while also being part of a collaborative team.

The Nurse Practitioner is responsible for delivering exceptional diagnostic and therapeutic patient care, adhering to the highest standards of our organization as part of a multidisciplinary team. Responsibilities include conducting initial and follow-up visits, as well as performing approved clinical functions autonomously.

The Nurse Practitioner fosters a patient-centric environment that addresses the holistic needs of individuals, including physical, psychosocial, spiritual, and functional aspects. This role involves collaboration with clinical staff and other healthcare professionals to promote optimal health and wellness for patients, families, and the community. The Nurse Practitioner may provide both autonomous and team-based care.

Education:

  • Master's Degree in Nursing required

Experience:

  • 2+ years of experience as a Nurse Practitioner required, with a strong preference for candidates with prior Cardiology and/or Heart Failure experience

Certification:

  • Certified New Hampshire Nurse Practitioner required upon hire
