Summary Statement

The selected candidate will be responsible for the assessment, referral, and case management of specialized court clients. These clients are located within the community and correctional facilities. Responsibilities include: maintaining case documentation, conducting urine monitoring for drug screenings, providing front desk support, referring clients to suitable levels of care, tracking client progress through weekly in-person meetings, reporting to the court during status hearings, conducting assessments and interviews within correctional settings, and assisting clients in accessing medical, housing, educational, and other essential services to support their needs. A hybrid remote work schedule is available.

Essential Functions

The essential functions outlined here are fundamental responsibilities common to all positions within this classification and are not intended to be an exhaustive list of all duties for any single position. This classification is distinct from a licensed Social Worker, as employees in this role do not perform the following tasks: diagnose individuals with mental, emotional, behavioral, addictive, or developmental disorders; conduct related client assessments or evaluations; provide psychotherapeutic treatment; or develop and implement assessment-based treatment plans. An employee in this classification may collaborate as part of a multi-disciplinary team to deliver behavioral health services or treatment plans in accordance with applicable regulations. Conduct interviews with clients, families, or professional sources to assess the client's needs for various health and human services, which may encompass employment, job search assistance, and basic skills development. Formulate a comprehensive, cost-effective service plan utilizing internal resources, referrals, contracted providers, or institutions (Service plans may include job training and employability, housing assistance, general public assistance, in-home services, and supports, or home-delivered meals, de-escalation techniques, peer services, or skill development). Perform ongoing monitoring and evaluation of service delivery to ensure optimal quality and efficiency; authorize and recommend necessary modifications and adjustments. Maintain detailed case records, summaries, statistical and narrative reports, and complete required documentation. Establish effective working relationships with clients, families, community members, and professional sources. Provide guidance and relevant information to connect clients with necessary services, which may include protective, adoption, and related services. Offer information and case coordination on-site, via email, or telephone to individuals or groups. Supply information and responses to inquiries regarding services for clients, families, community members, and government agencies. May provide supportive casework to clients prior to, during, or after hospitalization based on physician recommendations. Utilize automated information systems and software to research, review, evaluate, and report on health and human service data. Communicate and interact effectively with individuals across diverse cultures, abilities, genders, ethnicities, ages, and races. Perform other related duties as required.

Job Requirements

JOB REQUIREMENTS for Health/Human Service Case Manager II Candidates must possess education, training, and/or experience demonstrating competence in each of the following areas: Six months' experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs. Six months' experience in making recommendations as part of a client's service plan, such as clinical treatment, counseling, or determining eligibility for health or human services/benefits. Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures. Six months' experience in utilizing an automated information system to enter, update, modify, delete, retrieve/inquire, and report on data. Six months' experience in narrative report writing.
