Position Overview
We are currently seeking a full-time Nurse Practitioner II to join our Heart Failure Clinic team. This role will primarily involve working onsite during standard business hours, providing vital support to our cardiology services.
As integral partners to our physicians, Nurse Practitioners at WHP are empowered to practice autonomously within the scope of their education and certification while also collaborating closely with our multidisciplinary team.
The Nurse Practitioner will deliver outstanding diagnostic and therapeutic care, adhering to the highest standards of our organization. Responsibilities include conducting initial and follow-up patient visits, as well as performing approved clinical functions independently.
This role emphasizes a patient-centered approach, addressing the holistic needs of individuals, including their physical, psychosocial, spiritual, and functional requirements. The Nurse Practitioner will serve as a resource, collaborating with clinical staff and other healthcare professionals to promote optimal health and wellness for patients, families, and the broader community.
Qualifications

Educational Requirements:
- Master's Degree in Nursing is required.
Experience:
- A minimum of 2 years of experience as a Nurse Practitioner is required, with a strong preference for candidates with prior experience in Cardiology and/or Heart Failure.
Certification:
- Certification as a Nurse Practitioner in New Hampshire is required upon hire.
