Role Overview:

We are seeking a highly skilled and compassionate Senior Geriatric Care Specialist to join our team at Longevity Health.

About the Role:

This is a challenging and rewarding opportunity for a qualified Nurse Practitioner to work with seniors in long-term care settings. As a Senior Geriatric Care Specialist, you will be responsible for a panel of enrolled members receiving nursing home care. You will be supported by a dedicated clinical leadership team that provides education and guidance on best practices for effectively caring for our members.

Your Key Responsibilities:

  • Clinical Expertise: Provide collaborative primary care and services to a panel of Longevity Health Plan members, delivering high-quality care that addresses their unique needs and goals.
  • Advanced Care Planning: Develop and implement individualized care plans for each member, working closely with families and loved ones to ensure that their needs are met and their goals are achieved.
  • Nursing Facility Collaboration: Facilitate and administer Health Plan clinical operations functions, including prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promote membership growth.
  • Trusted Advisor: Serve as a trusted advisor to members, loved ones, and nursing home staff and colleagues, providing guidance and support to help them navigate complex healthcare systems.
  • Effective Communication: Deliver effective and frequent communication and coordination with members' PCPs, loved ones, and interdisciplinary care team members on members' care needs, goals of care, treatment plans/changes.
  • Autonomy and Accountability: Support overall market goals/targets and balance member caseloads, demonstrating autonomy and accountability in your practice.
  • Relationship Building: Establish meaningful relationships with members, loved ones, and nursing facilities, ensuring effective and high-quality care for Longevity Health members.
  • Additional Duties: Perform other related duties as assigned, supporting the overall mission and objectives of Longevity Health.

Requirements:

  • Education: Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program or Master's Degree from a nationally accredited program.
  • Experience: 6+ months experience as a Nurse Practitioner, with a strong preference for experience working in the nursing home environment.
  • Certifications: State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner required. Will allow acute care nurse practitioner with nursing home/geriatric or hospice palliative experience or state unrestricted licensed Physician's Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
  • Additional Requirements: DEA License required, active national provider number (NPI), active Medicare provider number, valid state driver's license with a good driving record, and proof of automobile insurance.

Estimated Salary: $120,000 - $150,000 per year, depending on location and experience.
