Hospice and Palliative Care Program Overview

Key Responsibilities

  • Triage and provide direct nursing care to Hospice patients during off-hours, including home visits and updates to patient plans.
  • Communicate effectively with RN case managers and physicians to ensure seamless care coordination.
  • Develop and maintain patient care records, leveraging technology to optimize documentation and care planning.
  • Collaborate with interdisciplinary teams to provide comprehensive care and support services.

Requirements

  • Graduate of an accredited nursing program with a valid RN license.
  • Minimum 1-2 years of experience in medical-surgical, community health, or hospice/oncology nursing.
  • Strong communication, listening, and teaching skills, with the ability to work effectively in a team environment.
  • Valid driver's license and reliable transportation.
