About the Role

We are seeking a skilled Clinical Liaison to join our team at Providence Service. As a Clinical Liaison, you will play a critical role in creating and maintaining positive customer relationships to increase referrals for Home Health, Hospice, and other services.

Key Responsibilities
  • Assess appropriateness for hospice and home health services and facilitate the coordination of services
  • Work with discharge planners, facility staff, physicians, and individuals to ensure seamless transitions
  • Develop and maintain strong relationships with healthcare providers and community partners
  • Ensure compliance with agency and statutory guidelines
Requirements
  • Education sufficient for clinical licensure (LPN/LVN) or Bachelor's Degree in Social Work
  • Current state professional license or certification appropriate to discipline
  • 3 years of experience in clinical discipline
Preferred Qualifications
  • Master's Degree in Social Work
  • Bachelor's Degree in Applicable clinical discipline (e.g., Respiratory Therapy)
  • Liaison, marketing or sales experience
  • Healthcare industry experience
