- Develop and maintain strong relationships with healthcare professionals and facilities to ensure seamless care transitions.
- Collaborate with interdisciplinary teams to identify patient needs and develop effective care plans.
- Provide education and support to patients and families to ensure successful care transitions.
- Education: Bachelor's Degree in Nursing or related field.
- Experience: 1 year of full-time experience in a clinical role.
- Licensure: Current RN license in the state of hire.
- Bachelor's Degree in Nursing.
- Liaison, marketing, or sales experience.
- Healthcare industry experience.
- Documented prior successful sales account medical management experience.
- Epic EMR experience.
