Key Responsibilities- Coordinate and arrange home care services between Providence Home Services and contracted facilities.
- Develop and maintain strong relationships with healthcare professionals and facilities to ensure seamless care transitions.
- Provide exceptional patient care and support to individuals in need of home health, home infusion, and home medical equipment services.
- Collaborate with our team to ensure timely and effective delivery of services.
- Stay up-to-date on industry developments and best practices to continuously improve our services.
- Education sufficient for licensure as a Registered Nurse.
- Washington Registered Nurse License or California Registered Nurse License (or equivalent in state of hire).
- 1 year of full-time experience in a clinical role.
- Bachelor's Degree in Nursing.
- Liaison, marketing, or sales experience.
- Healthcare industry experience.
- Documented prior successful sales account medical management experience.
- Epic EMR experience.
