Overview:

As a Care Navigator, you will collaborate with our Access and Admissions teams to facilitate connections to our diverse clinical mental health and substance use treatment programs. Your role will involve assessing various treatment needs and guiding individuals through the treatment process. Our services cater to adults, children, and families, making your contributions vital to our mission and community impact.

Work Schedule: Monday to Friday (40 hours), featuring rotating shifts - 3 days from 8:15 AM to 5 PM and 2 days from 10:15 AM to 7 PM. This position is primarily in-office, allowing for direct interaction with individuals receiving services.

Key Responsibilities:
  • Collaborate with the interdisciplinary clinical team and referral sources.
  • Guide individuals seeking treatment through the continuum of care.
  • Assist in completing enrollment documentation and scheduling future appointments.
  • Evaluate the holistic needs of individuals and ensure access to all relevant services.
  • Maintain a foundational understanding of mental health diagnostic criteria.
  • Support the Access Coordinator in data collection and system maintenance.
  • Comply with standards established by the Agency and funding sources to ensure quality service delivery.
  • Demonstrate proficiency in Microsoft applications and general computer skills.

Qualifications:
  • A Bachelor's degree in a Human Services-related field.
