GENERAL OVERVIEW:
Utilizing clinical expertise and available resources, the pharmacist is responsible for dispensing medications to patients in an outpatient pharmacy setting.

This role includes overseeing and actively participating in the compounding, preparation, and distribution of medications, ensuring compliance with all relevant state and federal pharmacy regulations.

The pharmacist addresses medication-related inquiries from both patients and healthcare providers, offering guidance on appropriate medication usage. Additionally, this position supervises pharmacy technicians.

KEY RESPONSIBILITIES:
The distribution of clinical and operational duties may differ based on the specific responsibilities assigned to the pharmacist.

For instance, a Level II pharmacist may have a crucial role in the primary outpatient pharmacy or an offsite community pharmacy, where opportunities for extensive clinical responsibilities may be limited.

Nonetheless, a higher level of specialized knowledge and competencies in their primary area of responsibility is anticipated compared to pharmacists in other departmental roles.


Medication Management and Distribution:

  • Comprehends and adheres to departmental and institutional policies regarding medication dispensing.
  • Accurately dispenses medications in a timely manner.
  • Independently resolves prescription issues promptly and appropriately.
  • Processes and validates prescriptions using current information system technology to maintain precise patient profiles.
  • Directs and oversees the work of pharmacy technicians to ensure effective workflow.
  • Communicates effectively and professionally with all hospital personnel.
  • Reviews all products requiring pharmacist approval prior to dispensing.
  • Engages in departmental quality assurance processes, including reporting and following up on medication incidents, reconciling medical administration records (MARs), and documenting medication adverse events and errors.

Clinical Engagement:

  • Develops or participates in clinical practices by collaborating with physicians, nurses, and other healthcare professionals, as well as patients, to recommend suitable medication therapies.
  • Involves in targeted clinical initiatives as defined by the department (e.g., immunization, discharge planning, transplantation, wellness screenings, etc.).
  • Monitors designated patient populations for clinical progress, including renal and hepatic function, white blood cell counts, and other relevant laboratory tests, recommending therapy adjustments as necessary.
  • Regularly documents clinical activities and interventions.
  • Contributes to the education of pharmacy staff and other healthcare professionals.
  • Identifies the need for patient education and conducts such sessions when required.

Problem Resolution:

  • Addresses issues through clear and direct communication with all relevant staff.
  • Proactively identifies patient care challenges and ensures resolution efforts are made before passing issues to the next shift.
  • Takes initiative in identifying barriers to patient discharge, including financial obstacles, and resolves them thoroughly.
  • Problem-solving may extend beyond patient-specific concerns, necessitating participation in interdisciplinary teams or revising hospital or departmental policies and procedures.
  • Performs the duties of a 'charge pharmacist' when scheduled, including coordinating shift resources, resolving staff conflicts, and ensuring effective communication between shifts.
  • Seeks assistance from supervisors when issues exceed their expertise or authority.

QUALIFICATIONS:

A Bachelor of Science degree in Pharmacy or Pharm D (preferred). A valid New York State Pharmacy License and an interest in outpatient practice.

The newly hired Level II pharmacist should possess either over one year of relevant ambulatory or community pharmacy experience or have successfully completed a Post Graduate-Year 1 (PGY-1) accredited pharmacy practice.

COMPENSATION:

Salary Range:
$126,256 - $163,613 Annually

The mentioned salary range represents the minimum and maximum compensation for this position.

Individual annual salaries or hourly rates will be determined within the job's compensation range, considering factors such as market data, education, experience, qualifications, and internal equity considerations.
