The salary range for this role is between $31,200 - $95,000, influenced by various factors such as the applicant's qualifications, relevant experience, educational background, certifications, and organizational needs.

Role Overview

The Clinical Pharmacist for Transitional Care Services is tasked with performing medication reconciliation following patient discharge (MRP) through telehealth interactions for Oak Street Health patients and those linked to the Oak Street Health Risk-Bearing Entity (RBE) after transitions of care.

The pharmacist will identify potential medication-related issues and work to reduce readmission risks through patient education, therapy recommendations to providers, and coordination of care plans.

Key Responsibilities:
• Review discharge summaries and engage with patients, caregivers, providers, and pharmacies to complete medication reconciliation post-discharge (MRP).

• Assess post-discharge medication orders and review medication profiles to prevent drug interactions, duplications, and allergic reactions.

• Ensure the appropriateness of drug therapy based on the patient's diagnosis and promote the selection of cost-effective medications that achieve desired therapeutic outcomes.

• Collaborate on patient care plans with the patient's primary care physician or managing medical team.


• Provide recommendations to the managing medical team regarding therapies or alternative approaches based on relevant medical literature and clinical experience.

• Communicate recommendations clearly and appropriately, considering patient-specific factors such as age, weight, and organ function.

• Interview patients to gather medical history and assess responses to therapy.

• Analyze laboratory and patient-specific data to develop treatment plans and monitor therapy responses.

• Apply pharmacokinetic principles to create effective, safe, and economical pharmacotherapeutic regimens.

• Address therapeutic inquiries from physicians and other healthcare providers.


• Identify and resolve drug-related issues, including complications from drug therapy.

• Establish procedures to detect significant drug interactions and make recommendations to avoid them.

• Ensure continuity of care by collaborating with other health professionals and external health organizations.

• Educate patients and caregivers on proper medication administration, potential adverse reactions, and storage.

• Participate in performance improvement initiatives, including problem identification, data collection, solution selection, implementation, and evaluation; assist in maintaining departmental equipment and supplies, along with other assigned duties.

Qualifications:

We seek a motivated and experienced clinical pharmacist with:
• A Bachelor of Science in Pharmacy or Pharm.D. degree.

• A current and unrestricted Pharmacist license in the state of MI.

• Completion of a PGY1 pharmacy residency plus at least three years of experience in an ambulatory healthcare delivery organization, or completion of both a PGY1 and PGY2 ambulatory care pharmacy residency.

• Board Certification (BCACP, BCPS, BCGP, CDCES, etc.) is strongly preferred.

• Proficiency in calculating medication dosages based on pharmacokinetic parameters.

• Ability to identify significant drug interactions.

• US work authorization.
