The Clinical Pharmacy Technician (CPhT) will participate in completing medication histories upon patient admission.

These will be conducted through face-to-face patient interviews and by reviewing the electronic medical record for both VA and DoD beneficiaries.

The incumbent will be responsible for the filling, dispensing, procurement, storage, preparation, review, distribution, and control of medications/other pharmaceutical products to patients.

Major duties and responsibilities includes but are not limited to:

Clinical Practice:
Conduct face to face appointments for medication reconciliation and discharge counseling under supervision of a clinical pharmacist.

Provide patient education on pharmacy topics including prescription refills, patient rights and responsibilities, and education specific to medication adherence.

Routinely review computer medication profile for prescription duplication, therapeutic duplication, prescription noncompliance, and allergies / adverse drug reactions. Reports identified medication related concerns to assigned clinical pharmacist for evaluation.
Communicate and follow-up on requests in a timely manner based on the needs of the clinic team /supervisor(s).

Report all potential patient or pharmacy distribution problems verbally and/or in writing as requested by the clinical pharmacist or pharmacy specialist in the area assigned.

Prioritize operational and assigned clinical functions to provide the highest quality of patient care.

Program Management:


Evaluate and analyze the workflow to coordinate activities in support of the Acute Care Clinical Pharmacist Practitioners and the transitions of care pharmacy team.

Ensures productivity is maintained at the established level for the programs for which the individual is responsible.
Assisting with medication use evaluations (MU-Es) and/or assisting with formulary management/prior authorization consults as assigned.
Assists in data collection and analysis for drug utilization, medication errors, adverse drug events and performance improvement initiatives.

Clinical Education and Scholarly Activities:
Completes all required Talent Management System (TMS) training on a timely basis.
Maintains knowledge and completes continuing education in therapeutic areas of expertise.
Assists in training new employees. Maintain competence by participating in staff development programs.
Provides in-service programs for the appropriate clinic personnel and pharmacy technicians as assigned.

Medication Processing and Distribution:
Fill VA/DoD prescription medications for window.

Performs routine cleaning on equipment (computers, automated dispensing devices, Laminar Workflow Bench, Biological Safety Cabinet, Restricted Access Barrier Systems, crash cart trays, Anesthesia carts and general work areas).

Ensures all first dose, unit dose cart fills, and ward stock fills are checked, per policy, prior to delivery to patient care areas.

Compounds and/or reconstitutes oral or external medications (non-sterile). This includes calculating and converting to proper weights and measures the amount of drug or solution to be used.

Perform additional duties as assigned.

Work Schedule:
Monday-Friday 7:30am to 4:00pm (CST), rotating every third weekend.
