This role is essential in identifying, preventing, and resolving medication-related issues. The Clinical Pharmacist provides expert information to healthcare professionals, patients, and the community, ensuring that the clinical, informational, and drug distribution components of pharmacy services operate cohesively to deliver patient-centered care.

Vision: The Department of Pharmacy aspires to be recognized nationally as a leader in advancing pharmacy practice through excellence, innovation, education, and research.


Key Responsibilities:


1. **Medication Order Interpretation:** Ensure the safety and proper use of medications by interpreting medication orders, being vigilant about compatibilities, incompatibilities, medication duplication, allergies, and potential administration errors.

2. **Problem Resolution Coordination:** Facilitate the resolution of medication-related issues among pharmacy, nursing, and prescribers, demonstrating initiative and sound judgment.

3. **Therapeutic Monitoring:** Monitor medication therapy for significant interactions and intervene when necessary, utilizing pharmacodynamics and pharmacokinetic expertise.

4. **Evaluation and Documentation:** Assess and document the appropriateness of selected medications for achieving desired therapeutic outcomes, communicating potential issues and solutions to prescribers.

5. **Consultation Services:** Act as a consultant in pharmacokinetics and adverse reactions for healthcare professionals, participating in therapeutic drug monitoring and intervention programs.

6. **Electronic Health Record Management:** Accurately process medication order information into the Electronic Health Record, maintaining comprehensive patient profiles.

7. **Medication Distribution Oversight:** Oversee the packaging, labeling, and distribution of medications, ensuring compliance with established policies and guidelines.

8. **Controlled Inventory Management:** Monitor the use of scheduled medications and maintain controlled inventories in dispensing areas.

9. **Pharmaceutical Compounding:** Compound and dispense pharmaceuticals, including sterile products and chemotherapy, ensuring accuracy and completeness in medication orders.

10. **Laboratory Data Review:** Review laboratory data for medication appropriateness and recommend changes as needed.

11. **Education and Training:** Develop and present educational programs for medical, nursing, and pharmacy staff, as well as educate patients and their families about medications.

12. **Emergency Response:** Respond to emergency situations to assist with medication management.

13. **Documentation of Activities:** Maintain accurate records of clinical activities and interventions performed, ensuring compliance with departmental requirements.

14. **Quality Improvement Participation:** Engage in quality improvement and medication use review activities.

15. **Workload Management:** Organize and prioritize workload effectively, collaborating with fellow pharmacists and directing the activities of the work area.

16. **Compliance Assurance:** Ensure adherence to hazardous waste collection and disposal procedures.

17. **Continuous Professional Development:** Maintain proficiency and expertise through ongoing professional development.


Qualifications:
Required:
- Current license to practice pharmacy in the State of Illinois
- Bachelor's Degree in Pharmacy or Doctor of Pharmacy Degree

Preferred:
- Specialized training in areas such as neonatology, critical care, or oncology
- Completion of a pharmacy residency program or equivalent hospital pharmacy experience
