Schedule: Full-Time Days

Shift: Monday to Friday, 8 AM - 5 PM

The Role You Will Play: The Patient Care Advocate role is designed for outreach within AdventHealth Medical Group, focusing on assessing patients who require care for various medical conditions identified through data analysis. This position involves conducting chart reviews to determine patient eligibility for outreach, contacting patients, scheduling appropriate appointments, and ordering necessary lab or imaging studies to facilitate patient care. Coordination with patients' primary care providers and their teams is essential to ensure seamless care delivery. The Patient Care Advocate will also support and implement quality improvement initiatives within the Physician Enterprise, which may include data research, project planning, monitoring quality improvement efforts, individual case reviews, and providing education to team members, providers, and practitioners. This role will be instrumental in educating clinic staff and providers on Quality Measures and the processes necessary to meet these standards, as well as supporting the ongoing development and maintenance of performance and clinical outcome standards. Additionally, this position will help coordinate and implement a clinical competency program for all non-provider clinical staff in ambulatory clinic settings.

  • Facilitates training programs for end users and ensures the availability of proper training materials before and during sessions.
  • Provides medical literature research services to patients, families, or healthcare providers.
  • Engages with payors to negotiate beneficial solutions for patients.
  • Communicates effectively and courteously with patients and colleagues, demonstrating excellent verbal, written, and listening skills, along with critical analysis abilities.
  • Manages responsibilities such as following up with patients to ensure attendance at appointments and follow-ups.
  • Exhibits strong attention to detail, a commitment to accuracy and excellence, and possesses excellent communication and interpersonal skills along with demonstrated organizational capabilities.
  • Designs and implements programs or processes aimed at optimizing gap closure.

Qualifications

The expertise and experience you will need to succeed:
  • Minimum of three years of clinical experience required.
  • At least three years of experience with EMR systems required.
  • LPN - Must hold an active LPN license in the state of Georgia.
