HIV/AIDS Case Management Specialist

Position Summary:
The EIS Case Manager is essential in delivering extensive case management support to individuals recently diagnosed with HIV/AIDS or those re-engaging with healthcare services. This role focuses on empowering clients from the HIV/AIDS and LGBTQIA+ communities to effectively navigate the healthcare landscape and access vital resources through a holistic approach to case management.
Key Responsibilities:
- Manage applications and recertifications for medication assistance programs.
- Provide case management services to all clients qualifying for Early Intervention Services.
- Offer benefits counseling to assist eligible clients in accessing various public and private programs.
- Conduct client intakes and updates for those eligible for AARC/ASCEND services.
- Maintain regular follow-ups and perform needs assessments and care planning aimed at promoting client self-sufficiency.
- Coordinate and oversee medical appointments, ensuring adherence to prescribed treatments.
- Facilitate Health Education Risk Reduction initiatives for clients living with HIV/AIDS.
- Ensure continuity of care by coordinating medical services and monitoring treatment adherence.
- Conduct initial assessments to identify case coordination needs.
- Develop personalized, comprehensive care plans for clients.
- Ensure timely access to appropriate health and support services.
- Provide eligibility updates for emergency walk-ins or scheduled assessments.
- Perform additional administrative tasks as assigned by the Director of Case Management.
- Execute other duties as required.
Qualifications & Skills:
- Strong proficiency in assessing needs and developing care plans.
- Exceptional written and verbal communication abilities.
- Thorough understanding of community resources and services.
- Competence in data entry and navigating various systems.
- Bi-lingual in English and Spanish is preferred.
Education, Licensure & Experience:
- Preferred: Associate's or Bachelor's degree from an accredited institution.
- Minimum of one year of professional experience working with individuals affected by HIV/AIDS or similar health challenges.
Work Schedule:
- Standard hours are Monday to Friday, 8 AM - 4:30 PM.
- Flexibility in scheduling may be required.
