Job Overview

Position Summary:

The EIS Case Manager is essential in delivering extensive case management support to individuals recently diagnosed with HIV/AIDS or those re-engaging with healthcare services. This role focuses on empowering clients from the HIV/AIDS and LGBTQIA+ communities to effectively navigate the healthcare landscape and access vital resources through a holistic approach to case management.

Key Responsibilities:

  • Manage applications and recertifications for medication assistance programs.
  • Provide case management services to all clients qualifying for Early Intervention Services.
  • Offer benefits counseling to assist eligible clients in accessing various public and private programs.
  • Conduct client intakes and updates for those eligible for AARC/ASCEND services.
  • Maintain regular follow-ups and perform needs assessments and care planning aimed at promoting client self-sufficiency.
  • Coordinate and oversee medical appointments, ensuring adherence to prescribed treatments.
  • Facilitate Health Education Risk Reduction initiatives for clients living with HIV/AIDS.
  • Ensure continuity of care by coordinating medical services and monitoring treatment adherence.
  • Conduct initial assessments to identify case coordination needs.
  • Develop personalized, comprehensive care plans for clients.
  • Ensure timely access to appropriate health and support services.
  • Provide eligibility updates for emergency walk-ins or scheduled assessments.
  • Perform additional administrative tasks as assigned by the Director of Case Management.
  • Execute other duties as required.

Qualifications & Skills:

  • Strong proficiency in assessing needs and developing care plans.
  • Exceptional written and verbal communication abilities.
  • Thorough understanding of community resources and services.
  • Competence in data entry and navigating various systems.
  • Bi-lingual in English and Spanish is preferred.

Education, Licensure & Experience:

  • Preferred: Associate's or Bachelor's degree from an accredited institution.
  • Minimum of one year of professional experience working with individuals affected by HIV/AIDS or similar health challenges.

Work Schedule:

  • Standard hours are Monday to Friday, 8 AM - 4:30 PM.
  • Flexibility in scheduling may be required.
