Job Overview

Position Summary:

The Client Access Specialist is responsible for overseeing non-medical case management and navigation services for individuals affected by HIV/AIDS. This role includes assessing client eligibility for various support programs, ensuring adherence to care protocols, and linking clients to essential resources. As part of a collaborative team, the Specialist maintains confidential client records and monitors service outcomes, focusing on enhancing client access to healthcare and facilitating the HIV care continuum.

Key Responsibilities:

  • Assist the Client Access Coordinator in managing, training, and scheduling navigation services for clients.
  • Oversee a caseload of clients within a non-medical case management framework, adhering to established guidelines.
  • Facilitate referrals for healthcare services as per regulatory standards.
  • Gather all necessary documentation from clients living with HIV/AIDS who qualify for support services.
  • Coordinate appointments for clients, including walk-ins, and provide timely reminders.
  • Evaluate urgent client needs and conduct intakes for pressing issues as required.
  • Enter client data into a secure database and electronic record-keeping system.
  • Engage in program quality enhancement initiatives, assist in implementing necessary changes, and promote innovative practices in collaboration with the care coordination director.
  • Manage applications, recertifications, and reporting for relevant support programs.
  • Perform additional duties as assigned.

Qualifications & Skills:

  • Establish and maintain positive professional relationships with clients and colleagues.
  • Encourage a collaborative work environment that promotes teamwork.
  • Proficient in data entry and navigating electronic systems.
  • Excellent written and verbal communication abilities.
  • Familiarity with community resources and services.
  • Awareness and sensitivity towards diverse populations, particularly those affected by HIV/AIDS and the LGBTQIA+ community.
  • Understanding of social service principles and effective communication with staff, clients, and partners.
  • Deliver unbiased, competent, and ethical support services.

Education, Licensure & Experience:

  • Preferred: Associate's degree from an accredited institution.
  • Preferred: At least one year of experience working with or within the LGBTQIA+ community.
  • Preferred: At least one year of experience with individuals living with HIV/AIDS or chronic health conditions.

Work Schedule:

  • Standard hours: Monday to Friday, 8 AM - 4:30 PM.
  • Flexible scheduling may be necessary.
  • Occasional travel may be required.
