Job Overview

Position Summary:

The EIS Case Manager is essential in delivering thorough case management services to individuals recently diagnosed with HIV/AIDS or those re-engaging with healthcare. This role focuses on gathering all required eligibility documentation from qualified clients. The aim is to empower individuals living with HIV/AIDS and members of the LGBTQIA+ community to effectively navigate the healthcare landscape and access vital services through a comprehensive case management approach.

Key Responsibilities:

  • Manage applications and recertifications for medication assistance programs.
  • Provide case management services to all clients eligible for Early Intervention Services.
  • Offer benefits counseling to assist eligible clients in accessing additional public and private programs.
  • Conduct client updates and intakes for those eligible for AARC/ASCEND services.
  • Ensure regular follow-ups and complete needs assessments and care plans aimed at fostering client self-sufficiency.
  • Coordinate and monitor medical appointments and adherence to prescribed treatments.
  • Facilitate Health Education Risk Reduction initiatives for clients living with HIV/AIDS.
  • Oversee medical care coordination, follow up on treatments, and ensure adherence to medical advice.
  • Conduct initial assessments to determine case coordination service needs.
  • Develop individualized, comprehensive care plans.
  • Ensure timely access to appropriate health and support services, maintaining continuity of care.
  • Provide eligibility updates for emergency walk-ins or scheduled EIS assessments.
  • Perform other administrative tasks as assigned by the Director of Case Management.
  • Execute additional duties as required.

Qualifications & Skills:

  • Expertise in needs assessment and care plan formulation.
  • Strong written and verbal communication abilities.
  • Comprehensive knowledge of community resources and services.
  • Proficient in data entry and system navigation.
  • Ability to provide unbiased, competent, and ethical services.
  • Fluency in both English and Spanish is preferred.

Education, Licensure & Experience:

  • Preferred: Associate's degree from an accredited institution, though not mandatory.
  • Preferred: Bachelor's degree, though not mandatory.
  • At least one year of paid experience working with individuals living with HIV/AIDS or other serious health conditions.

Work Schedule:

  • Standard hours: Monday to Friday, 8 AM - 4:30 PM.
  • Flexibility in scheduling may be required.

  • Primarily in-person at one location, with potential for off-site work as necessary.
