Advanced Patient Care Coordinator

Penn State Health is seeking a highly skilled Registered Nurse (RN) to join its Telemetry team at its Enola, Pennsylvania location. This dynamic role offers the opportunity to provide exceptional patient care and contribute to the success of a leading healthcare organization.
Job OverviewThis is an ongoing nursing job with 36 hours per week, requiring 12-hour shifts on days. The successful candidate will work collaboratively as part of a multidisciplinary team to deliver high-quality patient care, ensuring optimal health outcomes for our patients.
- As a Registered Nurse in this position, you will assess patient health problems and needs, develop individualized care plans, and maintain accurate medical records.
- You will also administer medications, advise patients on health maintenance and disease prevention, and monitor patients' conditions, reporting any changes or symptoms to the healthcare team.
In addition to your clinical expertise, excellent communication skills are essential for building strong relationships with patients, families, and colleagues.
Key Responsibilities- Assess patients' physical and emotional health needs, identifying potential issues and developing strategies to address them.
- Develop and implement comprehensive care plans, tailored to each patient's unique requirements.
- Administer medications and treatments, monitoring patients' responses and adjusting care plans as needed.
- Communicate effectively with patients, families, and healthcare professionals, providing clear information and addressing concerns.
- Maintain accurate and detailed medical records, ensuring that all relevant information is up-to-date and easily accessible.
- A valid license to practice as a Registered Nurse in the state of employment or a multistate RN license through the interstate Nurse Licensure Compact is required.
- BLS certification must be completed prior to orientation.
