Job Overview

The Clinical Nutrition Specialist is tasked with evaluating the dietary needs and implementing suitable strategies to ensure optimal health outcomes for participants.

  • Conducts thorough assessments of nutritional requirements prior to program enrollment.
  • Responsible for performing initial, semi-annual, and annual evaluations. Comprehensive assessments may encompass:
    • Dietary recall
    • Current eating habits
    • Meal frequency and patterns
    • Support with meal preparation and grocery procurement
    • Body composition analysis
    • Weight tracking
    • Changes in appetite
    • Challenges with chewing and/or swallowing
    • Food sensitivities and preferences
    • Monitoring for food scarcity
    • Cultural and religious dietary considerations
    • Medical history
    • Review of laboratory results
    • Assessment of the need for nutritional supplements and/or pre-prepared meals
    • Follow-up on educational interventions
  • Evaluates and prescribes appropriate therapeutic diets.
  • Collaborates with participants, caregivers, and the Interdisciplinary Team to develop tailored care plans.
  • Assesses participants' readiness for dietary changes and implements personalized interventions, adjusting care goals as necessary.
  • Oversees meal service in the Day Health Center to ensure compliance with dietary recommendations. Communicates any changes in dietary requirements to the Interdisciplinary Team.
  • Works with Day Health Center Staff and other team members to identify participants facing challenges with chewing and/or swallowing, referring them for further evaluation as needed.
  • Provides counseling and education to participants and their caregivers to promote health and well-being.
  • Maintains and organizes updated lists of frozen meals and nutritional supplements.
  • Reviews and modifies all cycle menus as needed.
  • Coordinates care with external healthcare providers.
  • Maintains regular contact and follow-up with participants identified as being at high nutritional risk.
  • Identifies clinical signs of malnutrition.
  • Calculates dietary needs and coordinates with Home Infusion Company for participants requiring enteral nutritional support.
  • Understands and adheres to the Participant Bill of Rights, striving for high levels of participant satisfaction.

Qualifications:

  • A Bachelor's Degree in Nutrition or a related field is required.
  • Completion of a dietetic internship or equivalent practical experience, along with registration as a Dietitian by the Commission on Dietetic Registration.
  • Minimum of one year of experience working with older adults.
  • Participation in annual mandatory training sessions, including infection control, TB testing, safety protocols, and BLS training.
  • Possesses strong analytical skills to assess and resolve issues while developing effective intervention plans.
  • Ability to foster and maintain interpersonal relationships across departments.
  • Proficient in applying adult learning principles in planning and executing educational activities, including one-on-one instruction for participants and their families, as well as staff training.
  • Engages in quality improvement initiatives based on data analysis.
  • Ensures adherence to departmental and external standards for quality care through participation in professional meetings and review of national practice standards.
  • Engages in various committees as assigned.
  • Must demonstrate competency in standardized skills after a period of independent work.
  • Must be medically cleared for communicable diseases and have up-to-date immunizations after engaging in direct participant contact.

Work Environment:

  • Work conditions may vary, with potential exposure to communicable diseases in patient environments.
  • Driving may be required within the service area, with possible exposure to extreme weather conditions.
  • Frequent walking, bending, and lifting of up to forty pounds may be necessary in the execution of duties.
