Nutrition Specialist

The Nutrition Specialist is tasked with evaluating the dietary needs and implementing suitable strategies to ensure the well-being of participants.
- Conducts assessments of nutritional requirements prior to program enrollment.
- Responsible for performing initial, semi-annual, and annual evaluations. Comprehensive assessments may include:
- Dietary recall
- Current eating habits
- Meal patterns
- Support with meal preparation and grocery shopping
- Anthropometric measurements
- Weight tracking
- Changes in appetite
- Challenges with chewing and/or swallowing
- Food allergies and preferences
- Monitoring for food insecurity
- Cultural and religious dietary needs
- Medical history
- Assessment of laboratory results
- Need for nutritional supplements and/or prepared meals
- Follow-up on educational initiatives
- Assesses and determines suitable therapeutic diets.
- Collaborates with participants, caregivers, and members of the Interdisciplinary Team to develop tailored care plans.
- Evaluates participants' readiness for positive dietary changes and implements personalized interventions, adjusting care goals as necessary.
- Oversees meal service in the Day Health Center to ensure compliance with dietary recommendations. Communicates any changes in dietary needs to the Interdisciplinary Team.
- Works with Day Health Center Staff and other team members to identify participants facing challenges with chewing and/or swallowing, referring them for further evaluation as needed.
- Provides counseling and education to participants and their caregivers to promote health and well-being.
- Maintains an updated inventory of frozen meals and nutritional supplements.
- Reviews and modifies all cycle menus as necessary.
- Coordinates care with external healthcare providers.
- Maintains regular contact and follow-up with participants identified as being at high nutritional risk.
- Identifies clinical signs of malnutrition.
- Calculates nutritional requirements and coordinates care with Home Infusion Company for participants receiving enteral nutrition support.
- Understands, adheres to, and advocates for the Participant Bill of Rights, striving for high levels of participant satisfaction.
- A professional level of knowledge equivalent to a Bachelor's Degree in Nutrition.
- Completion of a dietetic internship or equivalent practical experience, along with registration as a Dietitian by the Commission on Dietetic Registration.
- Minimum of one year of experience working with older adults.
- Participation in annual mandatory training, including infection control, TB testing, safety protocols, and BLS training.
- Requires advanced analytical skills to assess and resolve issues and formulate effective intervention plans.
- Ability to foster and maintain interpersonal and interdepartmental relationships.
- Capability to apply adult learning principles in planning and executing educational activities; includes one-on-one teaching for participants and family members, as well as staff education in various settings.
- Engages in and/or leads quality improvement initiatives based on data analysis.
- Ensures adherence to departmental and external standards in delivering quality care by attending professional meetings and reviewing national practice standards.
- Participates in various committees as assigned.
- Must meet a standardized set of competencies before working independently.
- Must be medically cleared for communicable diseases and have all immunizations current before engaging in direct participant contact.
- Work conditions may vary, with potential exposure to communicable diseases due to the patient environment.
- Driving is required within the designated service area, with possible exposure to extreme temperatures.
- Frequent walking, bending, and lifting of forty pounds or more may be necessary in the execution of duties.
