The Clinical Nutrition Specialist plays a vital role in the healthcare team under the guidance of the Clinical Nutrition Manager. This position is responsible for executing the Nutrition Care Process (NCP) by conducting thorough nutrition evaluations, establishing nutrition diagnoses, and creating and implementing evidence-based nutrition interventions while monitoring participant progress.
Key Responsibilities
Participant Evaluation- Conducts comprehensive nutrition evaluations at enrollment and periodically thereafter, adhering to established guidelines.
- Employs effective screening tools to assess nutritional status.
- Utilizes medical history, laboratory results, medications, anthropometric data, and dietary history for assessment.
- Performs nutrition-focused physical examinations to identify malnutrition risks.
- Assesses psychosocial and behavioral factors affecting food access and preparation.
- Evaluates participant and family readiness to implement positive dietary changes.
- Collaborates with interdisciplinary team members as necessary.
- Identifies patterns and potential causes of nutrition-related issues.
- Documents nutrition diagnostic statements with supporting evidence.
- Develops tailored interventions targeting the root causes of nutrition problems.
- Assesses educational needs and provides personalized nutrition education and counseling.
- Coordinates nutrition care plans with the interdisciplinary team and other healthcare professionals.
- Utilizes monitoring techniques that respect participants' cultural and personal preferences.
- Evaluates the effectiveness of nutrition interventions and tracks progress.
- Monitors high-risk participants and provides timely follow-up.
Documentation and Compliance
Documentation- Responsible for accurate and timely documentation in the electronic medical record.
- Participates in care plan meetings as an essential member of the healthcare team.
- Communicates care plans effectively with team members and other healthcare providers.
- Promotes participant rights and strives for high satisfaction levels.
Qualifications
- Bachelor's Degree in dietetics, nutrition, or a related field.
- Completion of an accredited dietetics program.
- Eligibility for registration or successful completion of the registration exam.
- Ability to foster interpersonal relationships across departments.
- Strong analytical skills for problem-solving and intervention development.
- Participation in quality improvement initiatives based on data analysis.
- Commitment to ongoing professional development and compliance with standards.
