Under the supervision of the Clinical Nutrition Manager, the Clinical Nutrition Specialist plays a crucial role in executing the Nutrition Care Process (NCP). This involves conducting thorough nutrition evaluations, identifying nutrition-related diagnoses, creating and implementing evidence-based nutrition strategies, and assessing participant progress. The nutrition evaluation encompasses various aspects, including the identification of malnutrition and obesity, assessment of food insecurity, and the formulation of personalized goals and strategies for individuals with intricate health conditions. The Clinical Nutrition Specialist is essential to the Interdisciplinary Team (IDT) and must effectively collaborate with all team members.
Participant Care
Nutrition Evaluation
- Performs comprehensive nutrition evaluations at enrollment and semi-annually, adhering to CMS guidelines.
- Employs screening tools effectively to assess nutritional status.
- Utilizes medical history, lab results, medications, anthropometric data, and dietary history for a complete nutritional assessment.
- Conducts nutrition-focused physical examinations (NFPE) to assess malnutrition risk when necessary.
- Reviews psychosocial, functional, and behavioral factors influencing food access, selection, preparation, and physical activity.
- Assesses the participant and family's understanding, readiness for change, and motivation for behavioral modifications.
- Collaborates with other IDT members or healthcare professionals as needed.
- Identifies patterns and potential causes of nutrition-related issues.
- Documents the nutrition diagnostic statement using the PES format (problem, etiology, signs/symptoms) and provides supporting evidence.
- Develops individualized interventions targeting the underlying causes of nutritional problems.
- Evaluates educational needs and offers tailored nutrition education and counseling based on individual circumstances, including nutrient requirements and social factors.
- Coordinates the nutrition care plan with the IDT and other healthcare professionals as necessary.
- Implements monitoring and evaluation methods that respect participants' nutritional needs, cultural, religious, and ethnic considerations.
- Assesses the effectiveness of nutrition interventions and tracks progress towards desired outcomes.
- Monitors participants identified as high nutritional risk and provides timely follow-up based on clinical judgment.
Documentation
- The Clinical Nutrition Specialist is responsible for accurate, relevant, and timely documentation in the participant's electronic medical record, following CMS, PACE SEMI, and departmental standards.
- Participates in Care Plan meetings as a vital member of the interdisciplinary team.
- Effectively communicates the care plan with the interdisciplinary team and other healthcare providers as required.
- Understands, adheres to, and promotes the Participant Bill of Rights, striving for high levels of participant satisfaction.
- Possesses a professional level of knowledge typically acquired through a Bachelor's Degree in dietetics, nutrition, or a related field.
- Completed a dietetics program accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND).
- Registration eligibility and/or successful completion of the registration exam as established by the Commission on Dietetic Registration.
- Ability to foster and maintain interpersonal and interdepartmental relationships.
- Capable of applying adult learning principles in organizing educational activities, including one-on-one teaching and peer education.
- Requires advanced analytical skills for problem-solving and developing effective interventions.
- Completion of annual departmental competencies.
- Engages in quality improvement initiatives based on data analysis.
- Ensures compliance with departmental and external standards in delivering quality care through participation in professional meetings and committees.
- Involvement in various committees as assigned.
- Participates in mandatory annual training and screenings, including infection control and safety training.
- Must be medically cleared for communicable diseases and have up-to-date immunizations before engaging in direct participant contact.
- Work conditions may vary, with potential exposure to communicable diseases due to participant environments.
- Driving may be required within the service area, with possible exposure to extreme temperatures.
- Frequent walking, bending, and lifting of forty (40) pounds or more may be necessary in the execution of duties.
