Position Overview
The Admissions Nurse plays a crucial role in fostering and sustaining positive interactions with clients and referral partners within the community. This position is vital in ensuring that the Director of Patient Care Services (DPCS), Executive Director (ED), and Community Education Director are informed about community needs related to hospice education and service delivery.

Key Responsibilities
  • Develop and enhance access to Bristol Hospice Services in the communities served.
  • Communicate effectively with the DPCS, ED, and Community Education Director regarding initiatives that enhance hospice accessibility.
  • Assess and address the educational requirements of the community concerning hospice care and services on an ongoing basis.
  • Monitor perceptions of Bristol Hospice as a premier provider of hospice services among community members, customers, payers, and patients.
  • Facilitate communication with agencies and referral sources to ensure a seamless continuum of care.
  • Maintain an up-to-date understanding of hospice educational resources and share relevant information with organizational personnel.
  • Oversee the hospice referral process and serve as a resource for reviewing patient information to determine hospice care needs.
  • Coordinate with hospice agency leadership and referral sources to ensure effective care coordination and timely admissions.
  • Engage with Medical Directors or Attending Physicians regarding patient hospice eligibility and necessary documentation.
  • Conduct hospice evaluations and admissions, ensuring comprehensive assessments of patients and families to identify hospice needs.
  • Collaborate with the intake department to serve as a liaison among facilities, physicians, hospitals, and hospice programs.
  • Respond to inquiries and concerns from customers and referral sources as needed.
  • Work alongside the ED and DPCS to plan, implement, and evaluate educational programs.
  • Participate in the Quality Assurance Performance Improvement (QAPI) process as directed.
  • Provide professional nursing care by applying all elements of the nursing process.
  • Utilize health assessment data to establish nursing diagnoses and contribute to the patient/family care plan.

Qualifications
  • Graduation from an accredited nursing program.
  • A minimum of one year of nursing experience within the last three years or a bachelor's degree in nursing from an accredited program.
  • Current RN license in the state of practice.
  • Exceptional observational, verbal, and written communication skills.
  • Strong problem-solving abilities and basic math skills.
  • Willingness to travel up to 20% of the time and maintain flexibility in work hours.

Work Environment
  • Adherence to company policies and regulations governing hospice and homecare.
  • Compliance with harassment and discrimination policies.
  • Possession of a valid driver's license and insured vehicle.
  • Willingness to undergo background checks and drug/alcohol screenings.

Physical Requirements
  • Ability to perform essential nursing tasks, including lifting and carrying up to 50 lbs.
  • Capability to engage in prolonged periods of standing, walking, bending, and stretching.
  • Demonstrated ability to handle stressful situations and interact compassionately with patients and families.
