The Admissions Nurse plays a crucial role in fostering and sustaining positive connections with clients and referral sources within the community. This position is essential for ensuring that the Director of Patient Care Services (DPCS), Executive Director (ED), and Community Education Director are informed about community needs concerning education related to the hospice care philosophy and service delivery.
Key Responsibilities
- Develop and enhance access to Bristol Hospice Services within the communities served.
- Communicate effectively with the DPCS, ED, and Community Education Director regarding initiatives that promote hospice accessibility.
- Assess and continuously meet the educational needs of the community related to hospice care and services.
- Monitor perceptions of Bristol Hospice as a premier provider of hospice services among community members, customers, payers, and patients.
- Facilitate communication with agencies and referral sources to support a seamless continuum of care.
- Maintain an extensive understanding of hospice educational resources and share pertinent information with organizational personnel.
- Oversee the hospice referral process and act as a resource for reviewing patient information to ascertain hospice care needs.
- Coordinate with hospice agency leadership and referral sources to ensure effective care coordination and timely admissions.
- Engage with the Medical Director or Attending Physician to discuss patient hospice eligibility criteria and obtain necessary documentation.
- Conduct hospice evaluations and admissions, ensuring thorough initial assessments of patients and families.
- Respond to inquiries and concerns from customers and referral sources as required.
- Collaborate with the ED and DPCS to plan, implement, and evaluate in-service and continuing education programs.
- Participate in the Quality Assurance Performance Improvement (QAPI) process as directed.
- Provide professional nursing care utilizing all elements of the nursing process, including health assessment data to establish nursing diagnoses.
- Educate patients and families regarding their care plans.
Qualifications
- Graduate of an accredited nursing program.
- Minimum of one (1) year of professional nursing experience within the last three (3) years or a bachelor's degree in nursing from an accredited program.
- Current RN license in the state.
- Excellent observational, verbal, and written communication skills.
- Strong problem-solving abilities and nursing skills as per competency checklist.
- Ability to travel 20% of the time and maintain flexible work hours.
- Willingness to keep up-to-date with information systems and relevant software.
Work Environment
- Compliance with company policies, procedures, and hospice regulations is mandatory.
- Must maintain a valid driver's license and have access to a reliable, insured vehicle.
- Willingness to undergo background checks and drug/alcohol screenings as required.
- Current CPR certification is necessary.
Physical Requirements
- Ability to perform physical assessments and interact with patients effectively.
- Must possess the ability to lift and carry up to 50 lbs. and manage prolonged periods of standing, walking, and bending.
- Compassionate demeanor while maintaining professional boundaries with patients and families.
