Job Purpose

To assess referrals for suitability and facilitate the admission process, ensuring a smooth transition for patients and their families into the hospice program. The role includes managing patient care and family support to guarantee effective symptom management, education, medication oversight, and coordination of hospice services while maintaining excellent communication with the patient's healthcare provider.

Key Responsibilities

  • Coordinate the interdisciplinary care team for assigned patients, overseeing the execution of the care plan.
  • Deliver primary nursing care to hospice patients in various settings.
  • Assume full responsibility for the professional management of hospice patients' care related to terminal illnesses.
  • Educate primary caregivers on necessary skills for patient care at home.
  • Collaborate with physicians and team members to establish effective pain and symptom management strategies.
  • Conduct nursing assessments and admissions for new patients.
  • Maintain clinical records in compliance with federal, state, and agency regulations.
  • Supervise hospice aides and certified nursing aides, ensuring quality care through regular visits.
  • Participate in on-call responsibilities as required.
  • Engage in performance improvement initiatives and staff development programs.
