Your daily responsibilities will include:
  • Providing personalized assistance to patients and their families for daily activities
  • Helping hospice patients with personal hygiene, oral care, bathing, and changing bed linens as needed
  • Assisting patients with dressing and undressing
  • Recording vital health information such as temperature, pulse, respiration, and blood pressure
  • Assessing and reporting any changes in the patient's condition to the Case Manager
  • Supporting patients with mobility and exercise routines
  • Completing additional assigned tasks as required
Qualifications:
  • Strong commitment to delivering outstanding care to patients and families
  • Valid CNA certification
  • Ideally, 1 year of previous experience in hospice or home health settings
  • Possession of a valid driver's license and a reliable, insured vehicle
  • Current CPR certification
  • Excellent observational, verbal, and written communication skills
  • Willingness to take on additional relevant responsibilities as needed
