As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care, coordinating services, and ensuring a seamless transition for patients and families into our hospice program.
Key Responsibilities:- Case Management: Coordinate the interdisciplinary care team for assigned patients, develop and implement individualized plans of care, and ensure timely and effective delivery of services.
- Patient Care: Provide primary nursing care for hospice patients, including symptom management, patient and family education, and coordination of services.
- Communication: Maintain open and effective communication with patients, families, physicians, and other healthcare professionals to ensure comprehensive care and support.
- Documentation: Maintain accurate and timely documentation of patient care, including progress notes, treatment plans, and other relevant information.
- Supervision: Supervise and mentor hospice aides and certified nurse aides to ensure high-quality patient care and adherence to agency policies and procedures.
- On-Call Responsibilities: Participate in on-call rotations to provide 24/7 coverage and support for patients and families.
- Licensure: Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice.
- Education: Bachelor's degree in Nursing or related field.
- Experience: Minimum 2 years of experience in hospice or palliative care, with a focus on case management and patient care.
- Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Knowledge: Thorough understanding of hospice regulations, policies, and procedures, as well as current trends and best practices in hospice care.
