Job Purpose:
To evaluate referrals for suitability and manage the admission process, ensuring a smooth transition for patients and their families into the hospice program. The role also involves overseeing patient care to guarantee effective symptom management, education, medication oversight, and seamless communication with healthcare providers.

Key Responsibilities:
  • Coordinate the interdisciplinary care team for assigned patients and oversee the execution of care plans.
  • Deliver primary nursing care to hospice patients across various settings.
  • Assume full accountability for the management of care related to terminal illnesses.
  • Educate primary caregivers on necessary skills for patient care at home.
  • Collaborate with physicians and team members to ensure effective pain and symptom management.
  • Conduct nursing assessments and patient admissions.
  • Maintain clinical records in compliance with regulations and guidelines.
  • Supervise hospice aides and certified nursing assistants, ensuring quality care.
  • Participate in on-call responsibilities as assigned.
  • Engage in performance improvement initiatives and contribute to team meetings.
  • Provide skilled nursing care as outlined in the care plan.
  • Assist in developing interdisciplinary care plans for patients and families.
  • For patients in skilled nursing facilities, manage symptoms related to terminal illnesses.
  • Verify that care plans are being implemented effectively by facility caregivers.
  • Integrate care plans with hospice and facility teams to ensure cohesive patient care.
  • Supervise certified nursing aides and companion services for home care patients.
  • Perform additional duties as directed by management.

Licensure, Certification, and Educational Requirements:
