Position Overview
The Admissions Registered Nurse plays a crucial role in fostering and sustaining positive connections with clients and referral partners within the community. This role involves keeping the Director of Patient Care Services (DPCS), Executive Director (ED), and Community Education Director informed about community needs concerning educational initiatives aligned with the hospice care philosophy.

Essential Qualifications
  • Graduation from an accredited nursing program is mandatory.
  • A minimum of one (1) year of nursing experience within the last three (3) years is required, or a bachelor's degree in nursing from a program accredited by the National League for Nursing.
  • A valid RN license in the state is essential.
Core Competencies
  • Exceptional observational skills are necessary.
  • Strong verbal and written communication abilities are required.
  • Proficient problem-solving skills, basic math skills, and nursing competencies as per the checklist are essential.
Additional Requirements
  • Willingness to travel up to 20% of the time, often with little notice.
  • Flexibility in work hours is expected.
  • Commitment to maintaining an up-to-date understanding of information systems and relevant software applications.
Key Responsibilities
  • Communicate community initiatives that enhance hospice access to the DPCS, ED, and Community Education Director.
  • Continuously assess and address the educational needs of the community regarding hospice care and services.
  • Monitor perceptions of Bristol Hospice as a premier provider of hospice services within the community.
  • Facilitate communication with agencies and referral sources to ensure continuity of care.
  • Maintain a thorough understanding of hospice educational resources and disseminate relevant information to organizational personnel.
  • Support the hospice referral process effectively.
  • Act as a resource for reviewing patient information to determine hospice care needs and eligibility.
  • Coordinate with hospice agency leadership and referral sources to ensure timely admissions.
  • Engage with the Medical Director or Attending Physician regarding patient eligibility for hospice services and complete necessary documentation.
  • Conduct hospice evaluations and admissions as required.
  • Ensure effective communication with referral sources and hospice agency leadership.
  • Perform initial assessments of patients and families to identify hospice needs.
  • Facilitate admissions as necessary.
  • Conduct comprehensive physical assessments and document patient history.
  • Collaborate with the intake department to streamline the referral and admission process.
  • Address customer and referral source inquiries and concerns as needed.
  • Work with the ED and DPCS to plan and evaluate in-service and continuing education programs.
  • Participate in the Quality Assurance Performance Improvement (QAPI) process as directed.
  • May function as an RN Case Manager, managing an assigned patient caseload.
  • Deliver professional nursing care utilizing all elements of the nursing process.
  • Utilize health assessment data to formulate nursing diagnoses.
  • Participate in interdisciplinary team meetings to develop patient/family care plans.
  • Provide education to patients and families related to their care plans.
  • Adhere to accepted professional standards and practices.
Work Environment

  • A valid driver's license and insured vehicle in good working condition are necessary.
  • Willingness to undergo background checks and drug/alcohol screenings is required.
  • Current CPR certification must be maintained.
Physical Requirements
  • Ability to observe color changes in patients' skin, listen to heart and lung sounds, and assess skin temperature.
  • Demonstration of vital sensory abilities, including vision and hearing sensitivity, is necessary.
  • Manual dexterity and the ability to manipulate medical equipment safely are required.
  • Effective communication with patients, families, and healthcare colleagues is essential.
  • Physical capability to lift and carry up to 50 lbs. and perform various physical tasks is necessary.
  • Ability to manage stressful situations and provide compassionate care to grieving families is crucial.
