Position Overview
The Admissions Nurse plays a crucial role in fostering and nurturing positive relationships with clients and referral sources within the community. This role is essential in ensuring that the Director of Patient Care Services (DPCS), Executive Director (ED), and Community Education Director are kept informed about community needs concerning hospice education and service delivery.

Key Responsibilities
  • Develop and enhance access to Bristol Hospice Services within the communities served.
  • Communicate effectively with the DPCS, ED, and Community Education Director regarding initiatives that promote hospice access.
  • Assess and address the educational needs of the community related to hospice care on an ongoing basis.
  • Monitor perceptions of Bristol Hospice as a high-quality provider of hospice services.
  • Facilitate communication with agencies and referral sources to ensure a seamless continuum of care.
  • Maintain a thorough understanding of hospice educational resources and share pertinent information with relevant personnel.
  • Oversee the hospice referral process and provide consultations regarding patient needs.
  • Coordinate with hospice agency leadership and referral sources to ensure timely admissions.
  • Engage with the Medical Director or Attending Physician to confirm patient eligibility for hospice services.
  • Conduct initial assessments of patients and families to identify hospice care needs.
  • Provide comprehensive physical assessments and histories of current and past illnesses for referrals.
  • Respond to inquiries and concerns from customers and referral sources as necessary.
  • Collaborate with the ED and DPCS on planning and implementing educational programs.
  • Participate in Quality Assurance Performance Improvement (QAPI) processes as directed.
  • Deliver professional nursing care by applying all elements of the nursing process.

Qualifications
  • Graduate of an accredited nursing program.
  • Minimum of one (1) year of nursing experience within the last three (3) years or a bachelor's degree in nursing from an accredited program.
  • Current RN license in the state.
  • Excellent observational, verbal, and written communication skills.
  • Strong problem-solving abilities and basic math skills.
  • Willingness to travel up to 20% of the time, often with little notice.
  • Flexibility in work hours and a commitment to maintaining knowledge of relevant information systems.

Work Environment
  • Adhere to company policies and procedures, as well as hospice and homecare regulations.
  • Foster positive working relationships and comply with harassment and discrimination policies.
  • Possess a valid driver's license and maintain an insured vehicle in good working order.
  • Submit to background checks and drug/alcohol screenings as required.
  • Maintain current CPR certification.

Physical Requirements
  • Ability to perform essential nursing tasks, including visual and auditory assessments.
  • Capable of lifting and carrying up to 50 lbs. and spending extended periods standing, walking, and bending.
  • Ability to manage stressful situations with compassion and professionalism.
