About the Job

As a Patient Care Navigator, you will be responsible for ensuring seamless transitions of care for patients throughout their healthcare journey. Your expertise and guidance will empower patients to take control of their health, making informed decisions about their treatment options and care plans.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with multidisciplinary teams, including physicians, nurses, and other healthcare professionals.

Key Responsibilities:

  • Develop and implement patient-centered care plans that address individual needs and preferences
  • Facilitate communication among patients, families, and healthcare providers to ensure timely and effective care
  • Collaborate with clinical teams to coordinate care transitions, including hospital discharges, home health services, and community-based programs
  • Provide education and support to patients and their families on disease management, self-care, and wellness strategies

Requirements:

  • Bachelor's degree in Nursing or related field
  • Current licensure as a Registered Nurse in Pennsylvania or eligibility for multistate licensure
  • At least 2 years of experience in nursing, with a focus on patient navigation, case management, or similar roles
