About the Healthcare Navigator Role

Penn State Health is seeking a highly skilled Registered Nurse to fill a dynamic role as a Nurse Navigator in our Emergency Department.

Job Summary

This position involves providing exceptional patient care and support throughout their healthcare journey, ensuring seamless navigation through our health system. As a Registered Nurse Navigator, you will be responsible for coordinating care, identifying discharge needs, and recommending alternative locations of care when necessary.

Key Responsibilities:
  • Care Coordination: Work closely with scheduling coordinators, physicians, and other healthcare professionals to ensure efficient patient flow and optimal outcomes.
  • Discharge Planning: Identify patients' discharge needs and services, and coordinate referrals to appropriate providers or facilities.
  • Nurse Navigation: Serve as a clinical resource, providing guidance and support to patients, families, and caregivers throughout the healthcare continuum.
Requirements and Qualifications
  • Education: Currently licensed to practice as a Registered Nurse by the state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
  • Certifications: AHA BLS certification required before the end of the orientation period.
  • Experience: Registered Nurse with Emergency Department experience preferred.

The estimated salary range for this role is $85,000 - $110,000 per year, depending on experience and qualifications.
