Element Care is seeking a skilled Geriatric Care Nurse to join our PACE team. As a member of our interdisciplinary team, you will play a vital role in providing high-quality care to our participants. Your expertise in geriatric care will enable you to develop and implement comprehensive care plans that meet the unique needs of our participants.

Key Responsibilities
  • Provide direct care to participants, including assessments, care planning, and implementation.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination.
  • Monitor and report on participant health status and social functioning.
  • Coordinate medication management, including ordering and administering medications.
  • Provide education and consultation to participants and caregivers on care plans, nursing treatments, and medications.
  • Communicate effectively with participants, caregivers, and healthcare providers to ensure optimal care.
  • Participate in after-hour visits and be available for emergency situations.
Requirements
  • Registered Nurse currently licensed in the Commonwealth of MA.
  • Minimum of two (2) years experience with the geriatric population.
  • Proficient in the use of Microsoft Office tools.
  • Ability to travel locally on a frequent basis in a personal vehicle.
