Salary Range: $41,000 - $45,000


Position Summary:

The SCO Geriatric Support Service Coordinator (GSSC) plays a crucial role in the case management operations of the organization. Under the guidance of the Team Manager and in collaboration with SCO representatives, the GSSC is responsible for conducting initial and ongoing evaluations of clients' needs, determining eligibility for Boston Senior Home Care (BSHC) services, and identifying additional community resources.

Key Responsibilities:

  1. Conduct comprehensive assessments of elderly clients' functional and social requirements through home visits, considering their eligibility for services and community programs.
  2. Perform continuous evaluations of client needs by maintaining contact with clients, their families, and support networks, adjusting service plans as necessary.
  3. Develop tailored service plans based on assessments, seeking guidance for complex case management issues and high-risk client situations.
  4. Facilitate interdisciplinary consultations with SCO personnel to review service planning recommendations and secure necessary approvals.
  5. Monitor the quality and effectiveness of services provided by BSHC through regular communication with clients and their support systems.
  6. Maintain accurate and timely documentation of client interactions and required forms to ensure comprehensive client records.
  7. Advocate for clients by demonstrating knowledge of BSHC services and community programs, while enhancing resource awareness within the organization.
  8. Engage actively in team activities and organizational initiatives, fostering a collaborative and respectful work environment.
  9. Represent BSHC in outreach efforts to ensure all client groups have access to necessary services.

Work Schedule:

This is a full-time role, with working hours from Monday to Friday, 9:00 a.m. to 5:00 p.m.

Travel Requirements:

The position requires up to 75% travel to perform home visits, necessitating adaptability to various weather conditions.

Qualifications:

  • Bachelor's Degree is required.
  • Experience in social work (LSW) is preferred but not mandatory.

Background Check:
