Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our PACE team as a Geriatric Nurse Specialist. In this role, you will be responsible for providing high-quality nursing care to our frail elderly participants, working collaboratively with the interdisciplinary team to develop and implement comprehensive care plans.

Key Responsibilities
  • Direct Care Provision: Provide direct nursing care to participants, including assessments, care planning, and implementation of care plans.
  • Interdisciplinary Collaboration: Work closely with the interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each participant.
  • Medication Management: Coordinate medication management, including ordering, administering, and monitoring medication, as well as ensuring proper storage and disposal.
  • Communication and Education: Communicate effectively with participants, caregivers, and healthcare providers to ensure seamless care coordination and provide education on care plans, treatments, and medications.
  • Team Leadership: Provide leadership and guidance to the team, including training and oversight of students and new staff members.
  • Quality Assurance: Monitor and ensure compliance with established quality assurance and operational policies, procedures, and standards.
Requirements
  • Education: Currently licensed Registered Nurse in the Commonwealth of MA.
  • Experience: Minimum of two (2) years of experience working with the geriatric population.
  • Skills: Proficient in the use of Microsoft Office tools and ability to travel locally on a frequent basis.
