Job Summary:

The PACE RN Supervisor is a key member of the East Boston Neighborhood Health Center team, responsible for ensuring the effective coordination and delivery of PACE home care services. This role requires strong clinical background in geriatric care, with expertise in home health care management and dealing with elders and families.

Key Responsibilities:

  • Supervise and Coordinate Home Care Services: Work with the Home Care Director and Home Care Manager to ensure the delivery of PACE home care services, including home nursing, ADL and IADL support services, and other services as required.
  • Lead Interdisciplinary Teams: Participate in the PACE Interdisciplinary Teams (IDT) to provide assistance with oversight of health aide and nurse scheduling, and provide direct supervision of health aides.
  • Performance Improvement and Quality Assurance: Monitor quality of care and performance improvement initiatives to address concerns, deficiencies, or trends, and make reports through QAPI or other avenues as required.
  • Policy Review and Revision: Assist in policy review and revision, updating policies, procedures, and workflows as needed to ensure consistency with practice, changes in regulatory or other requirements.

Requirements:

  • Education and Experience: BS in Nursing or equivalent experience or certification, with a strong clinical background in geriatric care.
  • License and Certification: Current MA Nursing License.
  • Skills and Abilities: Expertise in home health care management, dealing with elders and families, and strong communication and leadership skills.
