Since 1985, the Boston Health Care for the Homeless Program has dedicated itself to providing equitable and dignified access to top-tier health services for individuals and families facing homelessness in the greater Boston area. Each year, we serve over 10,000 homeless individuals, ensuring they receive comprehensive health care ranging from preventive dental services to advanced cancer treatments. Our team of clinicians, case managers, and behavioral health specialists operates in more than 30 locations, catering to some of the most vulnerable yet resilient members of our community.

Role Summary:

We are currently seeking a geriatric specialist physician to join a newly established team dedicated to delivering high-quality, patient-centered care to older adults experiencing homelessness at various outpatient locations, including a stand-alone patient-centered medical home and potential shelter-based primary care clinics.

Work Schedule: Per Diem; as needed

Key Responsibilities:
  1. Support for Geriatric Team
    • Provide guidance to a multidisciplinary team comprising a nurse practitioner/physician assistant, a nurse, and a case manager in the clinical management of older individuals experiencing homelessness.
    • Engage in regular team meetings to discuss individual cases, overall strategies, quality improvement initiatives, and workflow development.
    • Assist the team in creating assessments, documentation templates, and other necessary tools.
  2. Patient Care
    • Deliver episodic care to patients as required by the team at BHCHP sites and potentially within their homes, adhering to BHCHP's clinical and administrative protocols.
    • Coordinate patient care with on-site clinical personnel at host facilities, including shelters, hospitals, and detox centers.
    • Refer patients for specialty, hospital, recuperative, or other necessary care, ensuring continuity of care through appropriate information sharing.
    • Conduct direct patient care, including clinical administrative tasks such as lab follow-ups, quality assurance, and completion of patient documentation.
    • Ensure comprehensive follow-up on all laboratory tests, notifying patients of results in accordance with BHCHP guidelines and providing follow-up for any abnormal results.
    • Educate patients on health promotion and disease prevention strategies.
  3. Collaborative and Educational Engagement
    • Offer support to BHCHP staff by providing adequate physician backup.
    • Facilitate education for peers and clinic staff through presentations in areas of expertise.
    • Participate in collaborative agreements with nurse practitioners and physician assistants to ensure the highest quality of co-managed clinical care.
    • Engage in the education of medical students and residents, serving as a mentor and role model.
    • Actively take part in departmental meetings, general staff gatherings, and other relevant meetings, including those focused on quality assurance and pharmacy.
    • Attend continuing medical education (CME) meetings regularly to maintain clinical competence as mandated by the appropriate State Medical Board.
  4. Administrative Duties
    • Maintain accurate medical records in a timely manner, following BHCHP guidelines and utilizing standard forms such as problem lists and medication lists.
    • Participate in peer review through regular chart evaluations.
    • Keep an active DEA certificate, medical license, and current hospital privileges.
    • Perform any other reasonable and appropriate tasks as requested by the Medical Director.
Qualifications:
  • M.D. licensed to practice in Massachusetts.
  • Board eligible or certified in family practice, internal medicine, or another relevant specialty.
  • Board eligible or certified in geriatrics.
  • Life support training certification (e.g., CPR, BLS, ACLS) from a course with a hands-on, in-person component.
  • DEA X-waiver is preferred.
  • Familiarity with addiction medicine and/or health care for the homeless programs is preferred.
  • Strong understanding of the complex health issues faced by homeless individuals, including substance use disorders, harm reduction strategies, and social determinants of health; awareness of the historical and current impacts of racism and xenophobia in healthcare settings is preferred.
  • Bilingual candidates are encouraged to apply, especially those fluent in Spanish and Haitian Kreyol, which are prevalent in our patient population.
  • Interest in collaborating within multidisciplinary teams to provide primary care to homeless individuals.
Compensation and Benefits:
  • The compensation rate is $106.00 per hour.
