Position Summary:
BHCHP is seeking a Geriatric Consultant Physician to join a newly established team focused on delivering high-quality, patient-centered care to older adults experiencing homelessness at various outpatient locations, including a dedicated patient-centered medical home and potential shelter-based primary care clinics.

Work Schedule: Per Diem; as needed

Key Responsibilities:

  1. Team Support:
    • Provide guidance to a multidisciplinary team, including nurse practitioners, nurses, and case managers, in the clinical management of older adults facing homelessness.
    • Engage in regular team meetings to discuss individual patient cases, strategic initiatives, quality improvement projects, and workflow development.
    • Assist in creating assessment tools, documentation templates, and other necessary resources.
  2. Patient Care:
    • Deliver episodic care to patients as required by the team at BHCHP facilities and potentially in home settings, adhering to BHCHP's clinical and administrative protocols.
    • Coordinate patient care with on-site clinical personnel at partner institutions, including shelters and hospitals.
    • Refer patients for specialty, hospital, recuperative, or other necessary care, ensuring continuity of care through appropriate information sharing.
    • Provide direct patient care, including clinical administrative tasks such as lab follow-ups, quality assurance, and completion of necessary patient documentation.
    • Ensure comprehensive follow-up on all laboratory tests, notifying patients of results in accordance with BHCHP guidelines and managing abnormal results with the highest standards of care.
    • Educate patients on health promotion and disease prevention strategies.
  3. Collaborative and Educational Duties:
    • Support BHCHP staff by providing adequate physician backup.
    • Facilitate education for peers and clinic staff through presentations in areas of expertise.
    • Engage in collaborative agreements with nurse practitioners and physician assistants to ensure optimal clinical care for co-managed patients.
    • Participate in the education of medical students and residents, serving as a mentor and role model.
    • Actively engage in departmental meetings, quality assurance sessions, and other relevant gatherings.
    • Attend continuing medical education (CME) meetings regularly as required by the State Medical Board to maintain clinical competence.
  4. Administrative Duties:
    • Maintain thorough and timely medical records in compliance with BHCHP guidelines, utilizing standard documentation forms.
    • Participate in peer review through regular chart evaluations.
    • Keep an active DEA certificate, medical license, and managed care credentials.
    • Perform any other reasonable duties as requested by the Medical Director.

Qualifications:

  • M.D. licensed to practice in Massachusetts.
  • Board eligible or certified in family practice, internal medicine, or a relevant specialty.
  • Board eligible or certified in geriatrics.
  • Certification in life support training (e.g., CPR, BLS, ACLS) from a course with a hands-on component.
  • DEA X-waiver is preferred.
  • Familiarity with addiction medicine and/or healthcare for the homeless programs is advantageous.
  • Strong understanding of the complex health issues faced by homeless individuals, including substance use disorders and social determinants of health, along with an awareness of the impacts of racism and xenophobia in healthcare.
  • Bilingual candidates are encouraged to apply, particularly those fluent in Spanish or Haitian Kreyol.
  • Interest in collaborating within multidisciplinary teams to provide primary care to homeless populations.

Compensation:
The compensation rate is $106.00 per hour.
