Supervisor of Case Management Services

POSITION OBJECTIVE:
The primary goal of this role is to oversee, train, and provide leadership to the case management team at Phoenix Service Corporation, specifically focusing on the coordination of services for individuals with disabilities. This position is crucial in ensuring that vulnerable populations receive the necessary support and guidance in navigating intricate service delivery systems.
ROLE AND RESPONSIBILITIES:
The Case Management Supervisor is accountable for the management of the Case Management division, reporting directly to the Supervisor of Case Management Services. This role necessitates a deep understanding of disabilities and related issues, alongside exceptional communication, organizational, and management capabilities. The supervisor will engage with a variety of stakeholders, including clients, families, service providers, and legal representatives, to facilitate effective service coordination.
Key Responsibilities Include:
- Providing leadership and mentorship to case managers, ensuring high-quality service delivery and adherence to best practices.
- Overseeing compliance with program regulations and maintaining up-to-date knowledge of policies from relevant agencies.
- Managing the intake process for service referrals, including tracking and assigning cases to appropriate case managers.
- Monitoring budgetary aspects related to case management services, ensuring efficient resource allocation.
- Facilitating team meetings to promote collaboration and share insights among case management staff.
- Finalizing and approving service plans, ensuring they meet the needs of clients while adhering to regulatory standards.
- Maintaining a manageable caseload, providing direct support to clients as needed.
ESSENTIAL FUNCTIONS:
The Case Management Supervisor will engage in the following activities:
- Assist clients in accessing essential services, ensuring informed choices are made.
- Coordinate individualized care plans that reflect client preferences and needs.
- Conduct regular assessments to monitor client health and safety.
- Respond promptly to any reports of abuse or neglect, documenting all actions taken.
- Maintain effective communication with all stakeholders, fostering positive relationships.
QUALIFICATIONS:
To be considered for this position, candidates must possess:
- Experience in waiver case management, preferably within the relevant counties.
- A degree in social work, psychology, or a related field, or equivalent experience in social services.
- Proficiency in relevant software systems and strong organizational skills.
- Excellent verbal and written communication abilities.
- A valid driver's license and the ability to travel as necessary.
WORK ENVIRONMENT:
This role requires a combination of office work and field visits, with a focus on maintaining a safe and supportive workplace.
