Position Summary
The Registered Nurse is responsible for planning, organizing, and directing hospice care, leveraging nursing expertise with a focus on community health education and experience. This role involves utilizing community resources to tailor services that address the unique needs of individuals and families in their homes and communities.
Key Responsibilities
Patient Care:
- Conducts initial, comprehensive, and ongoing assessments of patients and families to identify hospice needs, including a thorough physical assessment and medical history.
- Delivers professional nursing care by applying all components of the nursing process.
- Evaluates patient status by:
- Creating and initiating a care plan
- Regularly reassessing patient and family/caregiver needs
- Adjusting the care plan as necessary
- Implements the care plan and modifies it as patient needs evolve.
- Utilizes health assessment data to formulate nursing diagnoses.
- Develops a care plan with established goals based on nursing diagnoses, incorporating palliative nursing actions, and engaging patients and families in the planning process.
- Initiates preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed by the physician.
- Provides counseling to patients and families to address nursing and related needs.
- Offers health care instructions to patients based on assessments and care plans.
- Assists patients with daily living activities, promoting self-sufficiency and optimal comfort care.
- Acts as Case Manager when designated by the Clinical Supervisor, coordinating care for an assigned caseload.
Communication:
- Maintains accurate clinical documentation regarding patient conditions and care provided, including pain and symptom management updates.
- Communicates with physicians about patient needs and reports any changes in conditions, obtaining necessary orders.
- Collaborates with community health professionals to coordinate care plans.
- Educates patients and families on self-care techniques, providing medication, dietary, and other instructions as directed by the physician.
- Ensures a safe environment for patients.
- Facilitates continuity of care with family members and other team members.
- Works collaboratively with families and the hospice interdisciplinary team to address emotional needs.
- Participates in interdisciplinary team meetings.
Additional Duties:
- Participates in on-call duties as per policy.
- Ensures availability of necessary equipment and services.
- Supervises ancillary staff and delegates responsibilities as needed.
- Commits to personal growth and development, enhancing professional knowledge through continuing education and training.
- Completes assigned case responsibilities.
- Engages in quality assessment and performance improvement initiatives.
Qualifications
- Graduate of an accredited School of Nursing.
- At least 1 year of recent acute care experience in a healthcare setting, plus 1 year of recent Home Care experience.
- Current nursing licensure in the state and CPR certification.
- Bachelor's Degree preferred, with one year of hospice care experience.
- Management experience is not required, but responsible for supervising hospice aides.
- Strong observational, verbal, and written communication skills, along with problem-solving abilities and basic math skills.
- Physical capability to perform nursing tasks, including lifting and transferring patients.
- Must possess a valid driver's license and have access to a reliable, insured vehicle.
