Job Summary

Radias Health is seeking a highly skilled and compassionate Case Manager to join our Targeted Case Management team. As a Case Manager, you will play a vital role in providing person-centered services to promote recovery and community integration of adults with serious persistent mental illnesses and co-occurring substance use disorders.

Key Responsibilities
  • Treatment Planning: Develop and implement individualized treatment plans that are based on diagnostic and functional assessments, approved by clinical supervisors.
  • Service Coordination: Ensure access to needed medical, social, educational, vocational, and other necessary services for clients.
  • Community Support: Develop and implement individual community support plans for each client every 6 months, and schedule face-to-face visits as dictated by the plan.
  • Referrals and Access: Make culturally sensitive referrals and assist clients in gaining access to needed services.
  • Commitment Process: Participate in the commitment process, providing testimony in probate court, writing court reports, and monitoring stayed warrants and provisional discharge agreements.
  • Reporting and Documentation: File all legally required reports with Vulnerable Adults, the Ombudsman, and Child Protection.
  • Client Support: Assist clients with housing issues, interface with HRA and PHA staff, and provide education to clients regarding their illness.
  • Team Collaboration: Collaborate with, support, and provide education to family members and others interested in the client's treatment and welfare.
  • Professional Growth: Participate actively in ongoing professional growth and development, maintain professional behavior, and participate in supervision.
Requirements
  • Education: Bachelor's degree in behavioral sciences or related field, or 30 semester credit hours in behavioral health sciences, and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
  • Experience: 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
  • Other: Must have a car, auto insurance, and a driver's license.
