Role Summary:

The Hospice Certified Nursing Assistant (CNA) is essential in delivering compassionate support to patients facing terminal illnesses in their residences. This role emphasizes comfort, dignity, and emotional support during the final stages of life, operating under the guidance of the Hospice Clinical Nurse Manager and in partnership with the interdisciplinary hospice care team.

Key Responsibilities:

  • Direct Patient Care: Deliver personalized care in accordance with established care plans, focusing on hygiene, nutrition, mobility, and overall comfort for patients and their families.
  • Emotional Support: Provide empathetic assistance to patients and their families, addressing their emotional needs and preferences during this sensitive time.
  • Clinical Responsibilities: Monitor vital signs, communicate any changes in patient conditions to nursing staff, and assist in pain management strategies.
  • Team Collaboration: Engage with the interdisciplinary team to effectively execute care plans, contributing insights based on direct patient interactions.
  • Family Education: Inform families about basic caregiving techniques and advocate for the comfort of patients and peace of mind for their families.
  • Record Keeping: Ensure accurate and confidential documentation of patient care and observations.

Qualifications:

  • A compassionate demeanor and a strong commitment to empathetic patient care.
  • Willingness to travel as required by the role.

Physical Requirements:
