Key Responsibilities

The RN Case Manager (Hospice) is responsible for the comprehensive nursing management of the Plan of Care for each hospice patient.
  • Continuously assesses personal and professional performance, implementing necessary adjustments to enhance productivity and care quality.
  • Complies with and promotes the policies, procedures, goals, and objectives of hospice to ensure high-quality patient care.
  • Aims for autonomy and sound critical thinking in delivering patient care in a cost-effective manner without compromising quality.
  • Collaborates with other professionals to develop the patient's Plan of Care and implements treatment strategies grounded in scientific nursing principles that support physical, psychosocial, emotional, and spiritual health.
  • Demonstrates effective communication skills, both verbal and written.

Qualifications

Experience Requirements
  • A minimum of one year of clinical experience.

License Requirements
  • Current RN licensure in the state of practice along with one year of clinical experience.
  • Current CPR Certification.
  • Possession of a valid driver's license, vehicle insurance, and access to reliable transportation, or public transit.
