Key Responsibilities
The RN Case Manager (Hospice) is responsible for the comprehensive nursing management of the Plan of Care for each hospice patient.
- Continuously assesses personal and professional performance, implementing necessary adjustments to enhance productivity and care quality.
- Complies with and promotes the policies, procedures, goals, and objectives of hospice to ensure high-quality patient care.
- Aims for autonomy and sound critical thinking in delivering patient care in a cost-effective manner without compromising quality.
- Collaborates with other professionals to develop the patient's Plan of Care and implements treatment strategies grounded in scientific nursing principles that support physical, psychosocial, emotional, and spiritual health.
- Demonstrates effective communication skills, both verbal and written.
Experience Requirements
- A minimum of one year of clinical experience.
- Current RN licensure in the state of practice along with one year of clinical experience.
- Current CPR Certification.
- Possession of a valid driver's license, vehicle insurance, and access to reliable transportation, or public transit.
