Key Responsibilities:
- Direct Patient Care: Deliver personal care services as specified in the care plan, including assistance with hygiene, nutrition, and mobility, while fostering a supportive atmosphere for patients and their families.
- Emotional Support: Provide compassionate emotional assistance to patients and their families, recognizing and addressing their unique needs during end-of-life transitions.
- Clinical Assistance: Aid in monitoring vital signs and promptly report any changes in patient conditions to nursing staff, contributing to effective pain management and comfort strategies.
- Team Collaboration: Engage actively with the interdisciplinary team to implement care plans effectively, sharing insights based on direct patient interactions.
- Family Education: Instruct families on basic caregiving techniques and advocate for the comfort of patients and peace of mind for their loved ones.
