Liberty Health is seeking an experienced Hospice Registered Nurse to join our team. As a Hospice Registered Nurse, you will play a vital role in providing high-quality, patient-centered care to individuals with terminal illnesses.

Responsibilities:

  • Apply a working knowledge of nursing theories and concepts to provide comprehensive care to patients and their families.
  • Coordinate physical care of the patient with family, patient, physician, and Hospice team to ensure seamless care delivery.
  • Communicate effectively with patients, families, and healthcare team members to ensure clear understanding of care plans and goals.
  • Participate in Interdisciplinary Team (IDT) meetings and quality reviews to ensure high-quality care and patient satisfaction.
  • Collaborate with the Hospice team to develop and implement individualized care plans that meet the unique needs of each patient.

Requirements:

  • Current RN licensure in the state of practice.
  • Successful completion of a nursing education from an approved school of nursing.
  • At least two years of clinical nursing experience, with a minimum of one year of hospice nursing experience.
  • Knowledge of the framework, function, and organization of a home health and hospice care program.
  • Working knowledge of the principles and practice of nursing according to nursing practice act, agencies policies and procedures, and reimbursement guidelines.
  • 12 hours of continuing education annually.
  • CPR certification.
  • Ability to accept travel assignments and work flexible hours, including weekends and holidays.
