Hospice Care Registered Nurse

Position Overview:
The Hospice Registered Nurse plays a vital role within the interdisciplinary team, focusing on the comprehensive assessment of clients' physical, emotional, psychosocial, and spiritual requirements. This position is responsible for crafting tailored care plans and implementing necessary interventions to ensure optimal patient care.
Key Responsibilities:
- Facilitates the coordination of client care, collaborating with various team members including social workers, chaplains, in-home aides, physicians, and volunteers.
- Conducts thorough assessments of clients and their families within 24 hours of hospice admission.
- Delivers skilled nursing care in accordance with the interdisciplinary group (IDG) care plan.
- Educates clients, families, and caregivers on specific care techniques tailored to individual needs.
- Acts as an advocate for clients and families, maintaining open communication with healthcare providers and other necessary resources.
- Contributes to bereavement support services for clients and their families.
- Ensures accurate documentation and proper utilization of medications, durable medical equipment (DME), and supplies.
- Participates in weekly and interdisciplinary group meetings to discuss patient care.
- Develops and leaves a comprehensive care plan in the home for other staff and contract personnel.
- Monitors the recertification and reauthorization processes for ongoing care.
- Maintains precise documentation and follows through with the revocation process when necessary.
- Regularly updates the Hospice Administrator on client status and progress.
- Upholds confidentiality in all aspects of the role.
- Promotes a safe environment for clients and colleagues, ensuring personal safety.
- May assist in evaluating the performance of in-home aides and contribute to annual competency assessments.
- Participates in on-call rotations as required.
- Engages in performance improvement initiatives as needed.
- May assist in the orientation of new staff members.
- May serve as a mentor for newly hired staff.
- Represents the nursing profession positively within the community.
- May engage in community educational activities as necessary.
- Encourages and participates in ongoing professional development in nursing and palliative care.
- Performs additional duties as assigned.
Qualifications:
- Active nursing license in North Carolina.
- Ability to function effectively as a core member of the interdisciplinary group.
- Strong public speaking skills.
- Commitment to high-quality customer service.
- Associate Degree in Nursing (ADN) required.
- Preferred: Minimum of one year of experience in hospice or home health nursing.
- Excellent listening and assessment abilities.
- Familiarity with various computer programs is desirable.
Physical Requirements:
The role may require prolonged periods of sitting, whether while driving, attending meetings, visiting clients, or performing computer-related tasks. Frequent bending, reaching, lifting, and effective communication skills are essential.
