Job Summary

Key Responsibilities
  • Provide Patient-Centered Care: Utilize nursing theories and concepts to deliver personalized care that meets the unique needs of each patient.
  • Ensure Quality Documentation: Ensure that all care and documentation meet reimbursement guidelines and regulatory requirements.
  • Coordinate Patient Care: Collaborate with patients, families, physicians, and the Hospice team to coordinate physical care and develop comprehensive care plans.
  • Effective Communication: Communicate effectively with patients, families, and healthcare professionals at all levels to ensure seamless care coordination.
  • Team Collaboration: Work collaboratively with all agency personnel to achieve shared goals and objectives.
  • Quality Improvement: Participate in Interdisciplinary Team (IDT) meetings and quality review processes to ensure continuous quality improvement.
Requirements
  • Current RN Licensure: Possess current RN licensure in the state of practice.
  • Nursing Education: Have completed a nursing education from an approved school of nursing.
  • Clinical Experience: Have a minimum of two years of clinical nursing experience, with at least one year of hospice nursing experience preferred.
  • Knowledge and Skills: Possess knowledge of home health and hospice care program framework, function, and organization, as well as working knowledge of nursing principles and practice according to nursing practice act, agency policies and procedures, and reimbursement guidelines.
  • Continuing Education: Complete 12 hours of continuing education annually.
  • CPR Certification: Possess current CPR certification.
  • Travel and Call Responsibilities: Be able to accept travel assignments and weekend/evening and holiday call responsibilities.
