Position Overview:
The Hospice Registered Nurse plays a vital role within the interdisciplinary team. This nursing professional assesses the comprehensive needs of clients, including their physical, emotional, psychosocial, and spiritual requirements, and formulates tailored care plans and interventions to address these needs effectively.
Key Responsibilities:
- Facilitates the coordination of client care and services, collaborating with social workers, chaplains, in-home aides, physicians, and volunteers.
- Conducts thorough assessments of clients and their families within 24 hours of their admission to hospice care.
- Delivers skilled nursing services as outlined in the Interdisciplinary Group (IDG) care plan.
- Educates clients, families, and caregivers on specific care techniques tailored to individual client needs.
- Acts as an advocate for clients and families, maintaining consistent communication with physicians and other necessary resources.
- Engages in providing bereavement support to clients and their families.
- Documents medication usage, durable medical equipment (DME), and supplies accurately.
- Participates in weekly team meetings and IDG discussions.
- Creates and leaves a comprehensive care plan in the home for other staff and contract personnel.
- Oversees the recertification and reauthorization processes.
- Ensures accurate documentation and follow-up regarding the revocation process.
- Regularly updates the Hospice Administrator on client status and progress.
- Maintains confidentiality in all aspects of the role.
- Promotes safety for clients and colleagues in all environments.
- May conduct annual performance evaluations for In-Home Aides and assist with competency assessments.
- Participates in on-call rotations as needed.
- Engages in performance improvement initiatives as requested.
- May assist in the orientation of new staff members.
- May serve as a mentor for new nursing staff.
- Represents the nursing profession positively in all interactions.
- May participate in community education initiatives as required.
- Encourages and participates in ongoing professional development in nursing and palliative care.
- Undertakes additional responsibilities as assigned.
The Hospice Agency serves a broad geographic area, ensuring comprehensive care across multiple regions.
Qualifications:
- Current and active nursing license in North Carolina.
- Ability to function as a core member of the Interdisciplinary Group.
- Strong public speaking skills.
- Commitment to high-quality customer service.
- Associate Degree in Nursing (ADN) required.
- Preferred: Minimum of one year of experience in hospice or home health nursing.
- Excellent listening and assessment abilities.
- Familiarity with various computer applications is desirable.
Physical Requirements:
Ability to remain seated for extended periods while driving, attending meetings, visiting clients, or performing computer tasks. This role may involve frequent bending, reaching, lifting, and effective communication.
