As a Hospice Registered Nurse with Healthkeeperz, Inc., you will play a vital role in delivering high-quality patient care and support to individuals and their families in need of end-of-life care.
Key Responsibilities- Client Care Coordination: Collaborate with the interdisciplinary team to develop and implement comprehensive care plans that address the physical, emotional, and spiritual needs of clients.
- Assessment and Evaluation: Conduct thorough assessments of clients and their families within 24 hours of admission to Hospice, identifying areas of need and developing strategies for improvement.
- Nursing Care and Education: Provide skilled nursing care and education to clients, families, and caregivers, promoting independence and self-care.
- Advocacy and Support: Serve as a client and family advocate, maintaining open communication with healthcare providers and other stakeholders to ensure seamless care coordination.
- Bereavement Support: Offer emotional support and guidance to clients and families during the grieving process.
- Documentation and Compliance: Accurately document client information, medication management, and care plans, ensuring compliance with regulatory requirements.
- Team Collaboration: Participate in weekly meetings and IDG meetings to discuss client progress, share best practices, and address challenges.
- Quality Improvement: Contribute to performance improvement initiatives, identifying areas for growth and implementing evidence-based solutions.
- Education and Licensure: Active NC nursing license and ADN required.
- Experience: At least one year of hospice or home health nursing experience preferred.
- Skills and Abilities: Excellent communication and assessment skills, ability to work effectively in a team environment, and proficiency in computer programs.
